Visa and Documentation Checklist of Requirements

Particulars

  • Passport (Valid at least 6 months prior to departure date and have at least 2 blank visa page)
  • Application Form (A4 size)
  • Recent Photo 3pcs (4.5 cm x 4.5 cm) (white background) (write name and birthday at the back)
  • Birth Certificate in Security Paper issued by the NSO / PSA or Certified True Copy of Local Civil Registrar and duly authenticated by NSO (issued within 1 year)
  • Transcribed Birth Certificate from Local Civil Registrar is required when entries in NSO Birth Certificate are blurred or unreadable. - If Applicable
  • If the birth certificate is "late registration", please submit in additional, a baptismal certificate and school record (Form 137) from high school or elementary and School Yearbook (if possible) - If Applicable
  • If there is no record in PSA/NSO, please submit a "Certificate of Non-Record" from PSA together with the birth certificate from Local Civil Registrar. - If Applicable
  • Copy of PSA (NSO) CENOMAR or Marriage Certificate (issued within 1 year) (if applicant is married) - If Applicable
  • Daily Itinerary / Schedule in Japan (draft of day to day visit/activities in Japan)
  • Original Bank Certificate (valid at least 3 months from the date of issue)
  • Latest Income Tax Return (Form 2316)
  • Explanation letter to reason of No Income Tax Return (also attach copies of receipts from payments of client - If Applicable
  • Authorization Letter (for Visa Filing and Pick-up) - If Applicable
  • PRC / IBP ID - If Applicable
  • Proof of assets (i.e. land titles, billing statement, copy of pay slip) - If Applicable

Guarantor who lives in Japan who will shoulder part or all of the applicant's travel expenses

  • Invitation letter from Guarantor, specifying relationship to the applicant - If Applicable
  • Residence Certificate of the friend/acquaintance and guarantor in Japan - If Applicable

Guarantor who lives in the Philippines will shoulder part or all of the applicant's travel expenses

  • Invitation letter from Guarantor, specifying relationship to the applicant - If Applicable
  • Proof of relationship between applicant and guarantor (e.g. Birth Certificates, photos, phone call details, receipts, passport copies and explanation letter stating the relationship of the applicant.) - If Applicable
  • Bank Certificate (original) and Income Tax Return (ITR Form 2316) (Copy) - If Applicable

For Employees

  • Original Employment Certificate
  • Approved Leave of Absence - If Applicable

For Self-Employed

  • Copy of Business Permit from SEC or DTI - If Applicable
  • Copy of Business Permit of Mayor's Permit - If Applicable

For Student / Minor:

  • Original Affidavit of Support of Parents/Guardian - notarized - If Applicable
  • Copy of School I.D - If Applicable
  • School Certificate or Registration - If Applicable

For Retiree / Senior Citizen

  • Copy of Senior Citizen Card or Retirement Documents - If Applicable
  • Original Copy of Marriage Contract (if traveling with spouse) - If Applicable
  • Affidavit of Support if Senior is dependent - If Applicable
